Game Changers Mental Health & Substance Abuse Services understands that an individual’s path to recovery is just that, individual and varies from person to person. Our supportive services are tailored to meet the needs of the individual Client and play a critical role in health and recovery. Services are delivered by a team of dedicated professionals focused on accountability, stability, and relapse prevention. This service empowers clients with substance use histories to heal and recover from addiction while addressing post-incarceration trauma.
Many people with mental health and substance use disorders continue to lack access to services and, too often, become enmeshed in a cycle of costly justice system involvement post-incarceration. We provide individualized treatment and service plans using information obtained from the risk and needs screening and assessment process. We realize that the first hours, days, and weeks of reentry are critical and having resources in place during the initial phase of the transition is vital to the long-term success of the client.
